A disaster waiting to happen: 'Dangerous' Dombivli bridge to remain open for public
The Kalyan Dombivli Municipal Corporation (KDMC) seems to have an appetite for risk as it plans to keep the Dombivli road bridge open for traffic despite the IIT Bombay terming it as dangerous. KDMC has said that it will take all possible care and fix the bridge with guidelines from the IIT and railway engineers.
According to the report on the Dombivli road bridge, it was assessed on March 23 by a team of IIT Bombay. The report said, the super-structure and the sub-structure of the bridge were unsafe and it should be closed down. The report stated that the inspection found heavy corrosion, poor quality concrete, misaligned parapet and failure of expansion joints. The report suggested a 12-point rehabilitation programme recommending that the bridge should be shut till then.

Local MNS corporator Mandar Halbe said there will be chaos if the bridge is shut as it will almost lead to closure of the township's east-west connectivity. "The Thakurli and other road bridge options are not practical. A meeting of all officials was called and it has now been decided that the bridge will not be shut and instead be taken up for rehabilitation. An estimate will be drawn up in the next two months," he told mid-day.
Rehabilitation report awaited
Central Railway's Mumbai Divisional Railway manager's office said a report has been requested from the IIT Bombay regarding rehabilitation of the bridge. "A decision will be taken after the IIT submits their report. The railways will be undertaking works of conducting polymer treatment to the bridge slabs from the base," a CR official said.
When questioned about the bridge being called 'dangerous' by IIT, KDMC Commissioner Govind Bodke said that it won't be a problem. "We are undertaking immediate works to remove all unnecessary load like footpath slabs and cables, etc to lighten the weight for safety in the monsoon. In addition to this, the bridge will be shut for heavy vehicles and only light vehicles be allowed. Further, we will work in consultation with the IIT and close one lane at a time to repair the entire bridge."

Mumbai: At JJ hospital, doctors take on 'abusive professor'
While the plea of Dr Payal Tadvi's family about her harassment was unheard by Nair Hospital authorities, the Grant Medical College administration doesn't want to take such incidents lightly. Five resident doctors (medicine) attached to the Grant Medical College and JJ Group of hospitals, have complained against an assistant professor (orthopedic), for allegedly shouting and being abusive after his aged relative, a patient there, was allegedly not taken care of. An internal committee has been formed to inquire into the alleged incident.
The Assistant Professor, Dr Nadir Shah, has rubbished the allegations, saying they were made to divert attention from his complaint against the medicine unit resident doctors and its head. While the hospital dean Dr Ajay Chandanwale and MARD representatives are awaiting the internal committee's report on the complaint, on Friday evening he claimed the matter was sorted. Asked about the action taken, Dr Chandanwale said, "It is an internal issue, which we have sorted and it is confidential."

When contacted, Minister for Medical Education Girish Mahajan, said he will not take such incidents lightly. "I will personally look into this matter and ensure that such behaviour from seniors or lecturers is not tolerated," he said.

The complaint
In their complaint, the five resident doctors have objected to the behaviour of Dr Shah, who according to them, had come to their unit, no 9, on the morning of May 11, inquiring about a patient known to him. He allegedly used abusive, foul and derogatory language while speaking to the female residents. He later went to ward no 4, where he again allegedly used abusive language against the lecturer and male resident doctors. The entire happenings were documented verbatim in the complaint letter submitted to the dean of the hospital on May 13, by the medicine unit head Dr Wiqar Shaikh.
In the two-pages letter, which was signed by the five resident doctors, there are details of the happenings and the exact nature of abusive words that were allegedly used by Dr Shah. Another complaint letter was submitted to MARD JJ on May 15.
Committee formed
On the basis of the complaint, an internal committee including Dr Sanjay Surase (Medical Supt), Dr Monali Chopade (RMO), Dr Bharat Rathod (Associate Professor, Medicine) and Dr Sadanand Bhise (Associate Professor, Forensic), was formed. The committee has recorded statements of staff nurses and ward boys who were present at the time of incident. They have corroborated that the Assistant Professor was indeed loud, but have not stated that he was abusive.

MARD intervenes
Dr Umang Shandilaya, president JJ MARD, said, "This is the first time I have come across a complaint made by five junior residents. The use of such derogatory language by an associate professor is not acceptable. On Friday, me, MARD JJ treasurer Dr Vivek Waghela, and two of the aggrieved junior residents met the dean and in a fresh letter the duo stated that they want the hospital administration to warn the assistant professor against using such derogatory words, and issue a memo to him. The dean has accepted the request and has assured that he will do the needful."
'Didn't abuse'
Dr Nadir Shah rubbished the allegations. He said he had submitted his say to the committee. According to Dr Shah, "I was on leave, when a relative of mine was brought to JJ hospital at 8 am on May 11. When I went to the medical ward, I was surprised that there were no doctors and no treatment was being provided. I also found that unit head Dr Wiqar Shaikh was not available. I raised my concerns with the junior doctors but didn't use abusive language."
Dr Shah added, "The worst is that my relative passed away and if this has happened with us, I wonder how the poor patients are being treated. My relative had come walking and few days later we took her body from the hospital. Her son has complained against the concerned doctors with the hospital authorities and these allegations against me were made with ulterior motives."
This was rubbished by the unit sources who said that the patient was in bad condition when brought and put on ventilator support. "Also she was diabetic and had a renal problem. The residents did everything to save her life by following the protocol," said one of them. Honorary unit head Dr Wiqar Shaikh only said, "All the submissions have been made to the college dean, only he is authorised to speak to the media."
'Faculty must behave'
Dr Ketan Vagholkar, a consulting surgeon said, "Using filthy language has become a common habit with the faculty. As a result, trainee resident doctors use the same language while dealing with patients. This has led to sudden increase in violent incidences against doctors. The faculty needs to behave themselves and lead by example. The faculty requires training in communication skills."

Mumbai doctor's suicide: Dr Payal Tadvi refused help, fearing more bullying by seniors
Fearing further stigmatisation, Dr Payal Tadvi refused to seek psychiatric help to ease the anxiety and sleeplessness she was suffering from after being allegedly harassed by her seniors, a second-year student who resided on the same hostel floor as her has revealed. The student said Dr Tadvi was scared of being tagged as a 'psychiatric patient' by the seniors and thought they might use the tag to cite her as unfit for the course.
The second-year student said, "She was extremely anxious and also had trouble sleeping. The workload and the harassment had taken a toll on her mental health. We had advised her to consult with the hospital's department of psychiatry. But she was scared of being tagged as someone with a mental disorder by seniors."
'Had she taken help...'
mid-day has previously reported about how the seniors were not letting her participate in any deliveries and shouting at her in front of patients and juniors for no fault of hers. Because of this, the student said, Dr Payal was scared that if the three accused, Hema Ahuja, Ankita Khandelwal, and Bhakti Mehare, got to know that she sought counselling, they would have taken the opportunity to prove that she was unfit to pursue an MD in gynaecology.

"Seeking psychiatric help is still stigmatised, even among medical students. So, she couldn't approach the in-house psychiatric help and she didn't have time to seek help in private. Had she taken help from the psychiatric department, she would have survived," said another batch mate from her department.

PG students' anxiety
Dr Tadvi was not alone in feeling that way. According to a study, this is the case with almost all medical students across the state. The study titled, 'Stress Among Doctors Doing Residency: A Cross-Sectional Study At A Tertiary Care Hospital In The City Of Mumbai', published in 2015 in the National Journal of Community Medicine had suggested counselling and social support could help improve the physical and mental well-being of the post-graduation doctors.
The report also stated anxiety is the highest among post-graduation students in their first year. While the stress level for first year students was at 46.2 per cent, it was 29.2 per cent for second year students and 36.2 per cent for third year students. The average stress level among all 273 respondents was 37.3 per cent. Dr S R Suryawanshi, head of the department of community medicine, was also a part of this study.
A 2007 study conducted on medical students at Nair hospital revealed that 51.37 per cent of the surveyed students suffered from stress, while 66.05 per cent had been diagnosed with anxiety and 39.44 per cent had depression. This study was published in The Indian Journal of Occupational Therapy.

Seeking help outside
Several medical students who suffer from anxiety and depression refuse to take psychiatric help fearing stigmatisation. They prefer private psychiatric clinics. Dr Sagar Mundada, a city-based psychiatrist concurred, "It's true that medical students also stigmatize psychiatric counselling even though they need it on a regular basis. There is always pressure on medical students, which often makes them vulnerable to depression, anxiety and stress." "But they are too scared to seek counselling inside the hospital. So, either they don't approach doctors or even if they do, they go outside, to private clinics. On a monthly basis, I get around 10-15 medical students seeking counselling due to stress," he added.
BMC to strengthen anti-ragging committee
After corporators slammed the BMC in the Dr Payal Tadvi case for failing to strengthen the anti-ragging committee in civic medical colleges, civic chief Praveen Pardeshi said during a general body meeting on Friday that they have taken strict cognizance of the matter and suspended the people concerned. He also said the anti-ragging committee will be strengthened and necessary steps will be taken to ensure such incidents do not take place. Meanwhile, Rais Shaikh, the Samajwadi Party group leader in BMC, said there is an urgent need to bring reforms in the current working pattern of doctors in the civic hospitals. He'd also written to the commissioner earlier this week regarding the issue.
